For individuals with autism,other disabilities, their families, and caregivers.
What to expect:
-Flu vaccine for ages 3+
-Covid vaccine for ages 12+
-Sensory-friendly (quiet, low-light)
-Registration encouraged but not required
-Arrive no later than 5:45pm to receive a vaccination
-Assistance for Spanish speakers
-The Nonprofit Collaborative is ADA compliant
Gift cards for individualswho complete the brief follow-up survey
What to bring:
1. Completed pharmacy consent form (will be attached to confirmation email)
2. Insurance card
No health insurance? No problem!AUSOM will provide funding for the uninsured. Please indicate this as part ofyour registration or notify the clinic staff upon arrival. Pre-registration isencouraged.
