The Parents’Place of Maryland will share information on the process of moving a young adultwith a disability to an adult model of health care – with or without a transferto a new clinician. The goals of Health Care Transition are:
To improvethe ability of youth and young adults to manage their own health care andeffectively use health services
To ensure anorganized clinical process in pediatric and adult practices to facilitatepreparation, transfer of care, and integration into adult care
